What to Expect

Prior to any procedure involving anesthesia or sedation, we require written consent to ensure we have all necessary information and your approval. This Surgery and Sedation Consent Form also allows you to list any current medications, special instructions, and confirm pre-operative care guidelines.

Dear Client,

Your pet is scheduled for an upcoming surgery. To help make the day as smooth and stress-free as possible, we’ve prepared a few simple forms for you to review and sign in advance. In addition to this letter, the following forms are available on our website for your convenience:

  • Pre-Anesthetic Blood Testing Information Form
  • Surgical Information Sheet

Please take the time to review all forms thoroughly. If you have any questions, feel free to call or email us at (770) 228-4321 or drgriffinanimalcare@gmail.com. On the day of your pet’s surgery, you will be asked to review and sign an Authorization/Estimate Form. We’ll also confirm the best phone number to reach you during your pet’s procedure.

The night before surgery, please do not feed your pet any food or treats after 10:00 PM. Water is okay and may be left out. If your pet is currently taking any medications, vitamins, or injections, withhold the morning doses unless directed otherwise by the doctor.

When you arrive for your pet’s scheduled surgery, the doctor will be available to answer any remaining questions and will collect the completed Authorization Form.

If you have chosen to proceed with any recommended blood testing, these will be performed before surgery begins — unless already completed. Should any questions arise, the doctor may contact you using the number provided on the Authorization Form.

After surgery, we’ll review the discharge instructions with you verbally and provide a written copy. If you have any questions or need clarification at any point, please don’t hesitate to ask.
